Prenatal care is the care you get throughout your pregnancy. Complete your first prenatal care visit on or before 14 weeks of your pregnancy or within 42 days of your enrollment date.

Postpartum care is the care you receive after the birth of your baby to check on your physical and mental health. Complete your postpartum visit with your doctor within 7 to 84 days after having your baby.

$25 VISA gift card for childhood vaccines.

Vaccines help protect children against serious diseases. Your child should complete the following vaccine series on or before their second birthday:

  • 4 Shots for Diphtheria (DTaP)
  • 4 Shots for Pneumococcal (PCV)
  • 3 Shots for Polio (IPV)
  • 3 Shots of Haemophilus Influenzae Type B (HiB)

Keep your teen on track with preventive care! It can have a big impact on their immediate and lifelong health.

$15 VISA gift card for immunizations for preteens and teens.

Preteens and teens need vaccines too. They should receive one meningococcal on or between their 11th and 13th birthday, one Tdap on or between their 10th and 13th birthday, and two HPV vaccinations on or between their 9th and 13th birthday.

$25 VISA gift card for well visits for ages 12 to 21.

During the teen years, it’s important to continue having routine checkups. Preteens and teens should have an annual well visit to monitor their physical and emotional development.

By preventing problems, or identifying them early, you are more likely to live a longer, healthier life. Talk to your health care provider to find out which screenings are right for you – based on your age, gender, health status and family history.

$25 VISA gift card for adult preventive visit.**

Health care visits are an opportunity for individuals to receive preventive services and counseling on topics such as diet and exercise. These visits also can help them to address acute issues or manage chronic conditions. Adults ages 20 years and older should have an annual in-person or telehealth preventive visit with their provider.

$15 VISA gift card for breast cancer screening.

A mammogram is a safe, low-dose X-ray and the best way to detect and diagnose breast cancer. Women ages 50 to 74 should have an annual mammogram.

$15 VISA gift card for diabetes HbA1c testing.

The A1C test a simple blood test that measures your average blood sugar levels over the past 3 months. Adults ages 18 to 75 with diabetes (type 1 and 2) should have an annual hemoglobin A1C (HbA1c) test.

Diabetic retinopathy is best diagnosed with a comprehensive dilate eye exam. Adults ages 18 to 75 with diabetes (type 1 and 2) should have an annual retinal eye exam.

Recovery is possible. If you’re struggling with addiction, call our behavioral health team toll-free at 1-800 873-2246, TTY 711. Complete a minimum of three substance use disorder (SUD) treatment sessions and you could earn Walmart® gift cards.1

Start treatment today. Complete one session2 within 14 days of a new SUD diagnosis.

Staying substance-free takes effort. Attend two more sessions2 within 34 days of your first visit (a total of three sessions).

2A session is any visit that addresses your SUD diagnosis, including a medication dispensing event (one prescription of an amount lasting 30 days or less), or a medication administration appointment.
