- Step 2: Click on Additional Benefits.
- Step 3: Then select Member Perks.
- Step 4: Submit the member information form on the page2
- Step 5: You will receive your gift card in approximately 6-8 weeks.

Pediatric Preventive Care
For Infants, children and teens ages 0-17, make sure your child is growing up healthy.

1Health Plan of Nevada Medicaid can change or cancel this program at any time. This program is for current members only. Member rewards will be sent to the email on the attestation form. Members are responsible for lost or stolen cards.

3A session is any visit that addresses your SUD diagnosis, including a medication dispensing event (one prescription of an amount lasting 30 days or less), or a medication administration appointment.
